This membership is ONLY available to those referred directly by a medical professional and requires a proof of eligibility. Once the referral is received and processed, you will be invited to an Initial Assessment and issued with a proof of eligibility by a member of the Healthwise team. The membership can then be purchased and the eligibility document will need to be uploaded via your online account.
Note: a GP referral does not guarantee eligibility and if membership is purchased a refund will not be provided.
